主要研究背景及目的

Some research indicated that uric acid was an independent risk factor of stroke, and related to poor prognosis and high disability rate in patients with acute stroke. However, there are other studies suggested that uric acid was a powerful anti-oxidant that exerts neuroprotective effects by acting as a free radical scavenger in plasma. The effect of uric acid levels on severity and prognosis of spontaneous intracerebral hemorrhage(ICH) remains controversial. We aim to explore the correlation of uric acid levels with severity and outcomes among ICH patients.

方法

We derived data from China Stroke Center Alliance (CSCA) database. This analysis included 85705 Chinese ICH participants in the CSCA database with a mean SUA level of 309.2±122.4μmol/l and median(IQR) SUA level of 301(237-370) μmol/l. Only 1.4% of the patients missed UA levels .SUA was a scale variable but was categorized into two groups, those having normal and high SUA levels (hyperuricemia) as follows; normal SUA range was SUA ≤ 357 μmol/l in females and SUA ≤ 417 μmol/l in males while high SUA range was SUA > 357 μmol/l in females and SUA > 417 μmol/l in males. SUA was also divided into quintiles (mean SUA level per quintile range).Demographic data, risk factors, clinical characteristics ,and outcomes were collected. Multiple logistic regression was done to adjust the confounding effects of the risk factors of stroke severity and In-hospital mortality across the groups to investigate the effect of uric acid levels on severity and prognosis of ICH patients. Furthermore, subgroup analysis will be performed by age,DM,BMI,CKD…

结论

UA was a protective factor for stroke severity and in-hospital complications such as pneumonia, poor swallow function, gastrointestinal bleeding, and DVT . However, no significant evidence indicated that UA levels were predictive of the discharge disposition and in-hospital mortality after ICH.
