Post-stroke cognitive impairment (PSCI) was a common but underestimated complication of stroke. Previous studies have found a one-third prevalence of PSCI. Inflammation was increasingly recognized as a common risk factor for dementia and stroke, but the underlying mechanisms and pathogenic pathways remain unclear. However, few studies examined the association between inflammatory marker and PSCI, and determined whether the association was affected by etiology. It was essential to identify biological biomarkers that can predict the occurrence of PSCI to enable the prediction or prognosis of PSCI and provide conditions for early intervention. Furthermore, previous study indicated that intracranial atherosclerotic stenosis (ICAS) was associated with cognitive impairment by causing cerebral hypoperfusion. Whether inflammation and ICAS have an interaction and exacerbate cognitive impairment was unknown. In this study, we aimed to investigate the association between inflammatory marker and risk of PSCI in patients with ischemic stroke or TIA, and determine whether this association depends on ICAS.
This study enrolled patients with ischemic stroke/TIA who had baseline inflammatory markers measured and completed standard 3-month cognitive function evaluation. The association between each inflammatory marker level and PSCI were investigated with the use of multivariable logistic regression analysis or linear regression analysis, after adjusted for potential covariates in all patients and patients stratified by TOAST subtype. The interaction of inflammatory markers with ICAS was further analyzed.
Elevated IL-6 levels were independently associated with one-year cognitive decline after ischemic stroke and TIA.
