主要研究背景及目的

Multiple studies and meta-analyses have confirmed the association between gut microbiota-derived metabolite trimethylamine Noxide (TMAO) and risk of cardiovascular events in multiple cohorts. Given this, TMAO precursors, such as choline and carnitine, are also considered as risk factors for adverse events, but previous studies have shown that, the prognostic value of choline and carnitine depends on TMAO, whose prognostic value is attenuated upon addition of TMAO. However, unlike choline and carnitine, another TMAO precursor, trimethyllysine (TML), was found to have an independent effect on cardiovascular risk from TMAO. As we known, this interesting finding was just confirmed by two relatively small corhorts (the Cleveland Cohort [N= 530)]; the Swiss ACS Cohort [N= 1683]), and was not verified in other races (such as Asians). Moreover, it is unclear whether the role of TML also exists in stroke populations. More importantly, to date, the underlying mechanism of TML for adverse events is largely unknown. Herein, this study aimed to verify the association of TML with adverse events using China National Stroke Registry Ⅲ consisted of more than 10,000 ischemic stroke patients, and to investigate the possible mechanism of TML for adverse events.

方法

The associations of TML with outcomes were examined using Cox regression model. Kaplan–Meier product limit method was used to generate survival plots, and the significance of differences between groups was tested by the log-rank test.

结论

Elevated trimethyllysine level is associated with cardiovascular death among patients with ischemic stroke/ transient ischemic attack. Mediation analyses propose that trimethyllysine contributes to cardiovascular death through inflammation and renal function, suggesting a possible pathomechanistic link.
