卒中后认知障碍随时间会有波动，研究发现缺血性卒中后认知障碍随时间会有改善，如再认功能可能会有改善。血管性病变如腔梗、微出血等发现与认知功能下降相关，阿尔茨海默病相关病变，如皮层萎缩、海马萎缩及APOE风险基因等也会加重认知障碍，那么卒中后认知功能改善者与非改善者是否存在影像学的差异呢，目前尚无研究证实
