Background And Purpose

Background: YKL-40, also called as human Chitinase 3-like 1 immunoassay. is one of an inflammatory marker of ischemic cardiovascular and cerebrovascular disease. Increased blood levels double the risk of ischemic stroke [HR (95% CI): 1.99 (1.42-2.67)]. The mechanism may be related to thin plaque cap and plaque vulnerability. A study involved 296 people with carotid plaques found vulnerable carotid plaques in the YKL-40 high-level group was 2.653 times higher than that in the low-level group (95% CI 1.002- 7.023) after adjusting for multiple risk factors. However, the relationship between YKL-40 and ischemic stroke remains to be determined.

Method

Multivariate analysis explored the relationship between blood YKL-40 levels and ischemic stroke recurrence and functional prognosis: Logit model with multi-class response variables was used to adjust age, gender, smoking, blood pressure, blood glucose, blood lipids, CRP, intraplaque bleeding, etc. For possible confounding factors, multiple correction models are used. Model 1 uses crude rate; Model 2 uses adjusted age and gender. Model 3 adds proofreading smoking based on Model 2. Model 4 adds proofreading blood pressure, blood glucose, lipids, and CRP based on Model 3. Model 5 is based on model 3 with proofreading of NIHSS. Model 6 is based on model 3 with proofreading of C-reactive protein. To investigate the relationship between the levels of various inflammatory factors and the recurrence and functional prognosis of ischemic stroke.

Conclusion

The elevated YKL-40 at admission can potentially help predict death, functional prognosis after ischemic stroke, which may help further studies to explore the potential physiological and pathological mechanism including the effects of vulnerable plaque and collateral circu-lation.
