Acute disseminated encephalomyelitis (ADEM) is a serious, life-threatening immune-mediated demyelinating disorder of the central nervous system. However, the incidence and mortality of ADEM in China are lacking. We conducted the first nationwide ADEM survey to describe the incidence and mortality of ADEM in China.
The study is based on the National Hospital Quality Monitoring System, which covers all 1665 tertiary hospitals in China. ADEM-related hospitalizations were identified using ICD-10 code G36.8 or G04.0 in any of the discharge diagnosis fields from 2016 to 2018.
This study provides the first estimate of ADEM incidence across all age groups and an incidence map of 31 provinces in mainland China.
6978 hospital admissions for 3101 patients with ADEM were identified from 1st Jan 2016- 31s Dec 2018, of which 2265 patients were newly diagnosed. The age- and sex-adjusted incidence of ADEM was 0.054/100,000 (95% CI, 0.052-0.056), with 0.134 (95% CI, 0.126-0.143) in children. The peak age of incidence was 5-to 9-year-old. A male predominance was found in ADEM, with 0.058(95%CI, 0.054-0.061) in males and 0.050 (95%CI, 0.047-0.053) in females. The ratio was 1.16:1. During 2016-2018, 95 patients died and the overall mortality rate for ADEM patients was 1.7%.
