To evaluate the efficacy and safety of ticagrelor-aspirin in patients with moderate (NIHSS 4-5) ischemic stroke.
The THALES (The Acute Stroke or Transient Ischemic Attack Treated With Ticagrelor and ASA for Prevention of Stroke and Death) trial was a randomized trial conducted at 414 hospitals in 28 countries in 2018/2019. This exploratory analysis compared stroke patients with baseline NIHSS 4-5 to those with NIHSS 0-3.
Patients with a moderate ischemic stroke have consistent benefit from ticagrelor-aspirin versus aspirin alone as compared to patients with less severe ischemic stroke
The observed primary-outcome event rate in patients with moderate stroke was 7.6% (129/1,671) on ticagrelor and 9.1% (150/1,641) on placebo (HR 0.84, 95% CI 0.66 to 1.06); and 4.7% (158/3,359) on ticagrelor and 5.7% (190/3,312) on placebo in those with less severe events at presentation (HR 0.82, 95% CI, 0.66 to 1.01) (P for interaction=.88).
