Clopidogrel is a prodrug requiring oxidation by the hepatic cytochrome P450 (CYP450) system to generate an active thiol metabolite. Clopidogrel combined with aspirin, has become a recommended treatment for patients with transient ischemic attack or acute minor stroke. Recent studies indicate that genetic variation substantially impacts clopidogrel efficacy, especially the genes encoding CYP450. Cytochrome P450, family 2, subfamily B, polypeptide 6 (CYP2B6) encodes a member of the cytochrome P450 superfamily of enzymes and plays a vital role in the degradation of various endogenous metabolites. Previous study has indicated that CYP2B6 polymorphisms were associated with the risk of ischemic stroke, however, the association between CYP2B6 polymorphisms and the efficacy of clopidogrel treatment in patients with stroke or TIA remains unclear. Our study is aimed to estimate the efficacy of dual therapy of clopidogrel and aspirin compared with aspirin alone according to CYP2B6 genotypes in patients with minor stroke or transient ischemic attack.
The relationship between outcomes and polymorphisms of CYP2B6 were assessed using a Cox proportional hazards regression model.
In this post-hoc analysis of the CHANCE trial, we did not observe a significant difference in the efficacy of aspirin plus clopidogrel compared with aspirin in carriers versus non-carriers of CYP2B6-516 GT/TT or -1456 TC/CC genotype.
