Subsequent stroke was common, especially for the patients with minor stroke and transient ischemic attack. Thought the mechanisms that precede impending stroke and other vascular events are relatively unexplored, relationship between hsCRP and recurrent stroke has been studied. However, whether the role of hsCRP in recurrent stroke differed between TIA/minor stroke and disabling stroke was unknown.
Patients were categorized into 2 groups based on NIHSS of 3. The primary outcomes consisted of stroke recurrence and composite vascular events within one year. The associations of hsCRP with outcomes were analyzed in this two groups of patients using Cox proportional hazards models. Patients were further classified according to TOAST subtype to assess the effect of stroke etiology on the association between hsCRP and recurrent vascular event. In the sensitivity analysis, another cut-point of NIHSS of 5 was used.
Elevated levels of hsCRP were associated with an increased risk of recurrent stroke in minor stroke or TIA patients, irrespective of using a National Institutes of Health Stroke Scale (NIHSS) score of ≤3 (the highest quartile vs. the lowest quartile: adjusted hazard ratio, 1.48; 95% CI, 1.12–1.97; p = 0.007) or ≤5 (the highest quartile vs. the lowest quartile: adjusted hazard ratio, 1.45; 95% CI, 1.15–1.84; p = 0.002) to define minor stroke. Such association was more apparent in the large-artery atherosclerosis subtype
