Previous studies discovered the impact of early-age exposure to famine and malnutrition on multiple chronic diseases, such as hypertension1, diabetes mellitus2 or cardiovascular disease.3 According to the study performed in the Dutch famine birth cohort, investigators were unable to find evidence for increasing risk of stroke in prenatal famine-exposed subjects.4 It is still unclear whether exposure to famine at early-age (postnatal) would influence the age of first-ever stroke event or recurrence of stroke. The primary goal of this study is to investigate the association between childhood/adolescence exposure to 1959-1962 China’s Great Famine and age of first-ever ischemic stroke onset. The secondary purpose is to explore the impact of exposure to famine on recurrence of stroke.
Birthdate would be acquired by ID number of each subject. The study population will be divided into 4 groups according to different stage of life circle when exposed to famine.5 Baseline characteristics will be compared among groups. We will present the categorical variables as proportions, continuous variables as mean and SD. In the univariate analysis, Kruskal-Wallis test was used for comparisons of skewed continuous variables and ordinal variable. Categorical variables were compared with the 2 statistics or Fisher exact test as appropriate. Cox proportional hazard regression model and logistic regression model will be used to analyze the association of different exposure stage with onset age and stroke recurrence. Stratified analysis by sex, BMI (<24; 24-27.9; ≥28) and severity of famine (provinces that whose average mortality rate excess 50% during 1959-1962 were defined as severely-exposed to famine) will also be performed. A two-sided P value of less than 0.05 will be considered to reach statistical significance. All statistical analyses will be performed by using SAS version 9.4 software (SAS Institute Inc., Cary, NC, USA).
Concurrency of early-age famine exposure and diabetes mellitus was associated with a higher risk of stroke recurrence within 12 months, and adherence to evidence-based KPI did not reduce the risk significantly.
