The observation that smokers with acute ischemic stroke could have better outcomes than nonsmokers led to the term “smoking paradox.” The controversy of such a complex claim has not been fully settled. Analyses were conducted on a nationwide data set to evaluate and determine whether such a paradox truly exists.
Continuous variables were described as mean (standard deviation [SD]) for normally distributed data and median (interquartile range [IQR]) for skewed data. Categorical variables were presented as the number (percentage). Logistic multivariable regression analysis was carried out to examine the association between smoking and in-hospital outcomes. Subgroup analysis was performed. In addition, we conducted a propensity-score matched analysis to further confirm the association between smoking and in-hospital outcomes.
Smoking was associated with increased risk of adverse in-hospital outcomes among patients with AIS in contemporary practice, reinforcing the importance of smoking cessation in patients with AIS.
